This 30-minute phone call is a valuable opportunity for us to connect, thoroughly assess your situation and priorities, clarify the payment structure, and determine if we are the right match.

Welcome to our comprehensive care coordination meeting. During this session, we will assess your current situation, identify pain points, and explore opportunities for collaboration and improvement. Within 48 hours, I will develop a working plan that outlines our "action items". We will decide together which items I will complete and which you may be able to manage based your budget and needs.

Additionally, you will receive my contract, choose a retainer, and sign HIPAA release forms enabling me to access your relevant medical documentation and collaborate with your medical team.

I will then begin to request your records through fax or patient portals, create your medical binder, and work on the agreed upon action items. We will maintain regular communication and continuously evaluate your needs, priorities, and goals as your health improves and your healthcare journey gains clarity.

You may likely reach a place where my services are no longer necessary. You have a great grasp of your healthcare situation, new communication and collaboration skills, improved quality of life, a positive relationship with your providers and a clear path ahead. I can support you as much or as little as you need and have a lifelong relationship, or place your file in archive if/until you need us again!
